Alex Zhavoronkov and Insilico Medicine demonstrate biological age reversal using AI-designed drug rentosertib
Insilico Medicine, led by founder and co-CEO Alex Zhavoronkov, published a study in Nature Biotechnology showing that the AI-designed drug rentosertib reverses predicted biological age. The study analyzed proteomic data from 42 patients with idiopathic pulmonary fibrosis who were treated with rentosertib during a Phase IIa clinical trial.

Researchers applied six independently developed proteomic aging clocks to the patients' blood samples, and all six models consistently indicated that the treated patients were biologically younger than the placebo group. In the 30mg twice-daily dose, patients showed a biological age reversal of approximately 3 to 4 years at week four. The drug also demonstrated a dose-dependent improvement in forced vital capacity, a measure of lung function, while simultaneously reversing age-related protein-expression trajectories. The study establishes a framework for integrating aging biomarkers into standard disease trials to accelerate the discovery of longevity therapeutics. While the results are not yet a regulatory endpoint, the study provides a proof-of-concept for rentosertib as a dual-purpose therapeutic targeting both aging and idiopathic pulmonary fibrosis. The drug is currently in Phase III clinical development.
