San Francisco Municipal Transportation Agency attributes July 31 train collision to operator fatigue

The San Francisco Municipal Transportation Agency confirmed that a train collision on July 31 was caused by operator fatigue. Video footage obtained by NorCal Bodycam shows a Muni operator nodding off moments before the train rear-ended another on the L Taraval line near 47th Avenue and Wawona Street. While the impact shattered the front windshield, no injuries were reported as both trains were largely empty at the time. SFMTA Director of Transportation Julie Kirschbaum stated that the incident was unacceptable and that vehicle and infrastructure issues have been ruled out. The operator involved has been placed on non-driving status while the formal investigation continues. This incident follows a similar event last September where an operator appeared to doze off, causing a crowded train to accelerate unexpectedly. In response, the SFMTA has increased random video reviews and allocated more rest time for operators to proactively address fatigue. The agency is currently coordinating with the California Public Utilities Commission to determine if the issues are systemic.
